RN Case Manager Home Health

Price: $37.00-$39.00 per hour

Location: Salt Lake County

RN License in Utah

We are seeking a compassionate Registered Nurse to provide skilled nursing care to home health patients in their homes. You'll work closely with patients, families, and an interdisciplinary team to develop personalized care plans that promote independence, recovery, and quality of life.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications, treatments, and skilled nursing services.
• Monitor patient progress and adjust care as needed.
• Collaborate with physicians and the care team to coordinate treatment.
• Educate patients and caregivers on disease management and self-care.
• Complete accurate, timely documentation in compliance with regulatory standards.

Qualifications

• Current RN license in Utah is a MUST!
• Home health experience requested, but not required.
• Strong assessment, communication, and critical thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Knowledge of OASIS and home health documentation is a plus.
• Reliable, personal transportation required.
